Dr. Tarlow is updating his contracting status with the following health plans. If you have one of the plans that we have stopped contracting with: Check with your plan manager to find out if you are covered by “Out Of Network” benefits or decide to see us as a cash pay patient.
Effective June 26, 2012 he will be non participating with AmeriPlan, Beech Street, Corvel, Great West, some Humana HMO/other plans (check Humana Website), Integrated Health Plan, Prime Health, SCAN, Three Rivers, and USA Managed Care.
Medicare Patients
I want to continue to treat you for your knee problems. I enjoy caring for people that have Medicare coverage. I see and treat Medicare Beneficiaries in my office and at the hospital. Dr. Tarlow has opted out of Medicare effective 1/1/2011. He is NOT a participating physician with Medicare. Medicare has specific and strict rules regarding payments to Dr. Tarlow – He can NOT accept any payments from Medicare. There are no exceptions made by Medicare. Medicare will pay Dr. Tarlow ZERO dollars for his services. Supplemental, Secondary and Medicare Replacement Plans will pay Dr. Tarlow Zero dollars for his services. Medicare patients treated by Dr. Tarlow pay Advanced Knee Care directly at the time or prior to services being rendered. The cash fee schedule is the amount that you will pay directly to Dr. Tarlow for treatment rendered. If you are a Medicare Beneficiary and would like to be treated by Dr. Tarlow that would be wonderful – please call to schedule an appointment. All other services related to your care including hospital, anesthesia, assistant surgeon, physical therapy, outside Xray/MRI/CT scan and lab work IS covered by Medicare/Supplemental and paid for by Medicare/Supplemental at prevailing terms.
