FMLA, STD, and Work Release Form

Fax or e-mail or hand deliver any Family Medical Leave Act (FMLA), Short term disability (STD), or Work Release form to our office for completion and signature. Call or e-mail to confirm receipt of form. If we do not call or e-mail confirmation assume we did not receive form. Allow 10 business days for completion. Include $35 form fee payment with form.
